Congenital cysts lined by conjunctiva in the region of the frequent sheath of the superior rectus and levator palpebrae muscles have been described. A mucocele arising from the paranasal sinuses can be lined by respiratory epithelium, typically very attenuated, but is a unique entity (see below). Orbital meningocele and meningoencephalocele (see Chapter 60) these rare abnormalities could additionally be congenital or acquired. Congenital lesions come up from a presumed defective separation of neuroectoderm from surface ectoderm, resulting in a bony dehiscence with a "cystic" herniation of dura into the orbit, both alone (meningocele) or with brain tissue (meningoencephalocele). They often present as a congenital cystic swelling of the medial orbit extending onto the face, accompanied by telecanthus and, frequently, epiphora. Cystic lesions They may be mistaken for sinus mucoceles, dermoid cysts, or nasolacrimal duct mucoceles. Posterior encephaloceles herniate into the orbit through the optic foramen, orbital fissures, or a bony defect. Typically, the attention is displaced ahead and downward44 and the proptosis increases on straining or crying. Plain X-rays show enlarged foramina or a bony defect of the posterior orbit. Paranasal sinus mucocele the paranasal sinuses are of scientific relevance to childhood orbital illness and it is necessary to be acquainted with their development.
